Names and addresses of over 4,700 people (current and prospective
members) in the Society for Vascular Ultrasound database are available
on a one-time use only basis, either as a computer listing
or as mailing labels. Over 50% of the Society's members work in
hospitals, 35% are in private laboratories or physicians' offices,
and the rest are physicians, researchers, educators, and other health
care providers; approximately 25% are physicians.
- All orders must be made on this
form and must include an explanation of how the labels will
be used, as well as a copy (or draft) of the proposed mailing
material.
- Telephone orders will not be accepted.
- Requested labels will be sent ONLY to a mailing house.
- Cost is $105 per 1,000 names or portion thereof, for labels
provided electronically. (SVU Affiliated
Chapters may receive up to two complimentary sets of electronic
labels per year of names in their regions (your state and two
adjoining states.)
- All label orders must be pre-paid in full. No purchase orders are allowed.
- The base price includes one sort, either alphabetically or by
zip code. Additional sorts are $26 each, regardless of size of order, except
for state sorts which are $16 per state.
