- Letter of submission, including corresponding author’s name, address, telephone, fax, and e-mail numbers.

- Typing
- Entire manuscript double-spaced.
- At least 12-pitch font.
- Unjustified right margin.
- All pages numbered consecutively in the upper right corner.
- A new page to begin section (ie, title page abstract, text, references, legends, and tables).
- Only one side of the paper used.
- Title page must include
- Full name, initials, degrees, and affiliations for each author.
- Concise title of article.
- Name, address, telephone number, fax number, and e-mail address of author to whom correspondence is to be sent.
- Name and address of author to whom reprint requests should be sent.
- Name of conference (if any) at which paper was presented, eg, 26th SVU Annual Conference.
- Two or three keywords.
- Abstract that replaces the summary, should clearly state in 200 words or less the main factual points of the paper.
- Tables should be self-explanatory and should be cited in numerical order in the text using Roman numerals. Type each table on a separate sheet of paper and provide a brief title for each table and provide the last name of the first author on the reverse. Tables should supplement, not duplicate, the text.
- Figures
- Three original copies of illustrations (photographic prints or original computer-generated illustrations) must be included.
- Figure number and first author’s name must be affixed to a label and placed on the back of figure. Do not write or print on reverse side.
- Indicate the top edge of figure.
- Indicate clearly which, if any, prints are to be printed in color.
- A legend should be included on a separate page containing captions for all illustrations.
- References should be double-spaced on a separate sheet of paper and in numeric order as cited in the text. Abbreviate journal names according to Index Medicus.
- Submit the original and three high-quality photocopies of the manuscript, tables, and illustrations.
- Submit a 3.5” computer diskette that exactly matches the hardcopy including tables, illustrations, and computer-generated figures. The preferred format is Word for Windows.
